Impact of public hospital reform on outpatient expenses in a military hospital

Abstract:Objective To explore the influence of public hospital reform on outpatient expenses in a military hospital in order to provide evidence for the participation of military hospitals in the reform of urban public hospitals.Methods All the outpatients treated in this hospital between 2005 and 2015 were investigated. Descriptive analysis was performed using SAS 9.3. The main indexes included the constituent ratio of outpatients and mean expenses.Results The mean expenses increased year by year, with a CAGR of 9.7%. The majority of the outpatients were local residents, who accounted for 95.6%, and there was significant difference(Z=47.2066,P<0.0001) in medical expenses between local and military outpatients. Self-pay outpatients (84.0%) were dominating in outpatient service, followed by outpatients (10.3%) covered by urban medical insurance, and there was significant difference(χ2=41624.2737,P<0.0001)in medical expenses between outpatients who paid for health care differently. Male patients accounted for 54.4%, who outnumbered female ones. The difference between the two groups was of statistical significance(CMH=40.6312,P<0.0001). Most of the outpatients aged 35 to 59, accounting for 49.6%. There was significant difference (χ2=2701.3116,P<0.0001) in the age of these outpatients.Conclusions Regulations on health care enforced in this area can help control the excessive growth of medical expenses. The integrated civilian-military development is an effectual solution to the bottleneck problem facing military hospitals under the context of public hospital reform. effectual way to solve the bottleneck problem of military hospitals under the context of public hospital reform.
