Correlation between sex hormone levels and insulin resistance in middle-aged patients with benign prostatic hyperplasia

Abstract:Objective To analyze the correlation between sex hormone levels and insulin resistance in middle-aged patients with benign prostatic hyperplasia (BPH). Methods The clinical data of 135 middle-aged male patients with BPH who participated in physical examination in a top three hospital from January 2020 to December 2022 were retrospectively analyzed, and according to the level of the insulin resistance index (HOMA-IR), 55 cases in the BPH insulin resistance (IR) group (HOMA-IR>2.69) and 80 cases in the BPH non-IR group (HOMA-IR≤2.69) were included, respectively. General data, sex hormone indexes and glucose metabolism indexes were compared between the two groups. The correlation between sex hormone levels and glucose metabolism indexes was analyzed. Results The levels of FBG, INS and HOMA-IR in IR group were higher than those in non-IR group, serum T and FSH levels were lower than those in non-IR group, E2 and E2/T levels were higher than those in non-IR group. Serum testosterone (T) showed a significant negative correlation with HbA1c (r=-0.27,P=0.002), HOMA-IR (r=-0.23,P=0.007), and serum T was negatively correlated with fasting insulin (FINS) (r=-0.20,P=0.021). Serum estradiol (E2) was positively correlated with FINS (r=0.19,P=0.023) and HOMA-IR (r=0.18,P=0.041), and E2/T was significantly positively correlated with FINS (r=0.30,P<0.001) and HOMA-IR (r=0.30,P<0.001). Serum follicle-stimulating hormone (FSH) was negatively correlated with FINS (r=-0.25, P=0.004) and HOMA-IR (r=-0.25,P=0.004); serum prolactin was negatively correlated with fasting blood glucose (r=-0.21,P=0.013). Conclusions Insulin resistance may be related with the levels of various sex hormones in middle-aged BPH patients, which has some clinical value for BPH prevention.
