Comparison of primary suture and t-tube drainage after laparoscopic common bile duct exploration

摘要目的 比较腹腔镜下胆总管探查术(laparoscopic common bile duct exploration,LCBDE)后一期缝合与T管引流术的疗效。方法 回顾性分析2016-01至2018-01在武警安徽总队医院行LCBDE的80例患者资料,根据处置方法不同分为一期缝合组与T管引流组,各40例,比较两种手术方式相关临床指标及术后并发症的情况。结果 两组患者的手术时间、术中出血量、术后并发症、残余结石比较,差异无统计学意义(P>0.05);一期缝合组患者腹腔引流时间、住院时间分别为(4.18±0.75)d、(9.53±1.65)d,均短于T管引流组的(5.83±1.04)d、(13.90±0.90)d,差异均有统计学意义(P<0.05)。结论 LCBDE后一期缝合术安全有效,并且可减少患者住院时间。
Abstract:Objective To compare the therapeutic effect of primary suture and t-tube drainage after laparoscopic common bile duct exploration (LCBDE).Methods The clinical data of 80 patients who underwent LCBDE in Anhui Corps Hospital of Armed Police between 2016 and 2018 was analyzed retrospectively. According to surgical methods,40 cases were assigned to primary suture group while another 40 cases were assigned to t-tube drainage group. The clinical indexes of the two surgical approaches and postoperative complications were compared.Results There was no statistically significant difference between the two groups in the duration of operation, intraoperative bleeding volume, postoperative complications and residual stones (P>0.05). The duration of abdominal drainage and hospital stay were(4.18±0.75)d and(9.53±1.65)d respectively in primary suture group, compared with (5.83±1.04)d and(13.90±0.90)d in t-tube drainage group, so the difference was statistically significant(P<0.05).Conclusions Primary suture after LCBDE is safe and effective while shortening hospital stay.
