Risk factors for deep neck space infections |

Abstract Objective To analyze clinical characteristics of deep neck space infections(DNSIs), identify potential risk factors associated with life-threatening complications,and to explore the factors that lead to life-threatening complications of DNSIs. Methods The clinical data on 125 cases of DNSIs treated between January 2004 and March 2017 was retrospectively analyzed. The indexes included manifestations, infected spaces, etiology, bacteriology, life-threatening complications, and treatment plans. The potential risk factors for such life-threatening complications as airway obstruction, pneumonia, mediastinitis / mediastinal abscess, and septicemia were analyzed. Results Among the 125 cases, there were 39 cases of life-threatening complications, accounting for 31.2%. These life-threatening complications were due to dyspnea in 24 cases (61.5%), pneumonia in 16 cases (41.0%), mediastinitis/mediastinal abscess in 11 cases (28.2%), electrolyte disturbances in 5 cases (12.8%) , infectious shock in 3 cases (7.6%)and pyemia in 2 cases (5.0%). The other 86 patients were at low risk. Multivariate logistic regression analysis showed that diabetes mellitus, infections involving multiple spaces, and systemic inflammatory response syndrome (SIRS) were risk factors for life-threatening complications in case of DNSIs (P<0.05). Conclusions The risk factors for life-threatening complications of DNSIs include diabetes mellitus,infections involving multiple spaces and SIRS. It is critical to prevent and treat life-threatening complications so as to improve cure rates and reduce the mortality.
