Analysis of disease distribution in blood transfusion patients with positive results of direct antiglobulin test

Abstract:Objective To analyze the disease distribution in blood transfusion patients with positive results of direct antiglobulin test (DAT). Methods The DAT test results and clinical data of 1609 blood transfusion patients in the Department of Transfusion Medicine of General Hospital of PLA Northern Theater from March 2010 to April 2022 were retrospectively analyzed, the relationship between different types of diseases and test results was explored, then the composition ratio of DAT positive patients and the changes in Hb, RBC, and Hct before and after red blood cell transfusions between the DAT positive group and the DAT negative group were investigated, so as to compare the efficiency of red blood cell transfusion between the DAT positive group and the DAT negative group. Results Of the 1609 cases, 1113 were DAT negative and 496 were DAT positive. Among 496 DAT-positive patients, rheumatic immunity-related diseases accounted for the highest proportion at 71.17%,and hematologic diseases followed at 13.91%. Among rheumatic-immunity-related diseases, the connective tissue disease (21.57%) and systemic lupus erythematosus (19.56%) accounted for the highest proportion; among hematologic diseases, thrombocytopenia (3.83%) and anemia (3.02%) accounted for the highest proportion. There was no significant difference in Hb, RBC, or Hct between the two groups before and after red blood cell infusion. The effective rate of red blood cell transfusion was 57.89% in DAT positive group and 78.95% in DAT negative group,and the difference between the two groups was statistically significant (P<0.05). Conclusions DAT is of great significance in clinically determining the nature of disease and accurate blood transfusion.
