Abstract Objective To evaluate the therapeutic effect of microvascular decompression against primary trigeminal neuralgia and improve the surgical approach. Methods A total of seventy-six cases of patients with primary trigeminal neuralgia treated with microvascular decompression in our hospital between February 2012 and December 2015 were selected as subjects. All these patients were treated with the modified decompression method. Results Among the seventy-six cases, sixty-five were cured, eleven had their symptoms relieved, and three were invalid. Among the eleven cases whose symptoms were relieved, pain disappeared in eight cases after one month of rehabilitation and three cases were ineffective. The total effective rate of surgery was 96%. There were no neurological deficits or severe complications in any follow-up. No CSF leaks were observed and no death occurred. Conclusions Microvascular decompression is an reliable and safe treatment of primary trigeminal neuralgia. This modified method helps to improve the effective rate and reduce complications.
