Effect of probiotics on prolonged postoperative ileus after acute abdomen surgery

摘要目的 评价使用益生菌对急腹症术后延迟性肠麻痹(PPOI)的影响。方法 回顾性分析2018-01至2024-01在武警江西总队医院因急腹症行手术的56例患者的临床资料,按照治疗方式分为益生菌组和对照组,各28例。收集两组患者的基本资料,包括年龄、性别、手术类型、手术方式、手术时间及出血量。比较术后第1、7天的炎症反应、营养状况、肠道功能恢复情况和PPOI发生率的差异。结果 两组治疗后的的中心粒细胞计数、C反应蛋白、白蛋白差异有统计学意义(P<0.05)。益生菌组首次排气时间[(46.29±23.06)h vs. 64.29±27.25)h;P<0.05]和住院时间[(236.57±97.22)h vs.(314.14±151.59)h;P<0.05]短于对照组,PPOI发生率更低(10.71% vs. 39.29%;P<0.05)。结论 急腹症术后使用益生菌利于减轻炎症反应,促进患者肠功能恢复和减少PPOI发生。
Abstract:Objective To evaluate the effect of probiotics on delayed prolonged postoperative ileus (PPOI) in acute abdominalgia. Methods A retrospective analysis was conducted on 56 patients who underwent surgery of acute abdominalgia at the Hospital of Jiangxi Armed Police Corps from 2018-01 to 2024-01. They were divided into the probiotics group and the control group, with 28 cases in each group. General information including age, gender, surgical types, surgical methods, operation time and blood loss were collected. Differences in postoperative nutritional status, inflammatory response, intestinal function recovery, and the incidence of PPOI between the 1st and 7th day after surgery were compared. Results Comparing the neutrophil count (NEUT), C-reactive protein (CRP), procalcitonin (PCT), body weight (Wt), albumin (ALB), hemoglobin (Hb), and total lymphocytes(TLC) between the two groups on the 1st day after surgery, the difference was not statistically significant and the data were comparable (P>0.05). Comparing the effects of different intervention methods on the NEUT values, CRP values, and ALB values measured before and after, the differences were statistically significant (P<0.05). The first exhaust time[(46.29±23.06) h vs. (64.29±27.25)h; P<0.05] and hospitalization time [(236.57±97.22) h vs. (314.14±151.59)h;P<0.05] in the probiotics group were shorter than those in the control group, and the incidence of PPOI (10.71% vs. 39.29%;P<0.05) was lower than that of the control group (P<0.05). Conclusion Probiotics after surgery of acute abdominalgia is beneficial to reducing inflammatory response, promoting recovery of intestinal function and decreasing the occurrence of PPOI.
