Clinical correlation factors of serum osteocalcin levels in middle-aged and elderly men undergoing health checkups

Abstract:Objective To investigate the relationship between serum osteocalcin (OC) levels and biochemical markers of bone turnover, sex hormones, and blood test indicators in middle-aged and elderly men undergoing health checkups, and analyze the clinical correlation factors of serum OC. Methods A retrospective analysis was conducted on 793 cases of middle-aged and elderly men who underwent health checkups in hospital from January 2023 to December 2023, and the subjects was divided into a low OC group (OC<14 ng/ml) and a normal OC group (14 ng/ml≤ OC ≤ 46 ng/ml) based on serum OC measurement values. General clinical data, blood test indicators, sex hormones, and biochemical markers of bone turnover were collected, and the relationship between serum OC levels and various indicators was analyzed. Results There were 539 (67.97%) in the low OC group and 254 (32.03%) in the normal OC group. The average age, white blood cell count, fasting blood glucose, glycosylated hemoglobin A1c(HbA1c), and estradiol of the low OC group were higher than those of the normal OC group. The average total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, serum alkaline phosphatase (BALP), blood phosphorus, blood sodium, blood chloride, parathyroid hormone (PTH), type I procollagen N-terminal propeptide (PINP), and β-collagen specific sequence (β-CTX) were all lower than those of the normal OC group, with statistically significant differences (P<0.05). Serum OC was positively correlated with PTH (r=0.315,P<0.001),PINP (r=0.755,P<0.001),β-CTX (r= 0.761,P<0.001), BALP (r=0.340,P<0.001), and blood phosphorus (r=0.118,P=0.001). The results of multiple linear regression analysis showed that age (β=-0.16, P=0.001), white blood cell count (β=-0.151, P<0.001), HbA1c (β-.148, P=0.001), homocysteine, (β=0.083, P= 0.007), blood phosphorus (β=0.215, P<0.001), serum testosterone (β=-0.110, P=0.006), PTH (β=0.265, P<0.001), and BALP (β=0.319, P<0.001) could predict the change of serum OC. Conclusions Age, inflammation, sex hormones, and some biochemical markers of bone turnover are correlated with serum OC in middle-aged and elderly men, and controlling inflammation, maintaining a good blood glucose level, estrogen/androgen balance, and endocrine homeostasis are beneficial for maintaining healthy bone metabolism in middle-aged and elderly men.
